The undersigned ("Registrant") requests that the residence, house, apartment or other dwelling of Registrant identified below (the "Residence") be placed on the City's "Do Not Knock" Registry (the "Registry") pursuant to Codified Ordinance 840.021. Registrant acknowledges that, except as otherwise permitted by Chapter 840 of the Codified Ordinances or other law, placement of the Residence on the Registry will prohibit any canvasser from knocking on the door of, ringing the doorbell of or otherwise calling at the Residence. Registrant further acknowledges that, after being lawfully placed on the Registry, the Residence shall remain on the Registry until the earliest of any of the following:

The Director of Public Safety receives a Notice of Removal pursuant to Codified Ordinance 840.021(c); or
The Director of Public Safety receives notice that Registrant is not or is no longer a lawful possessor or occupant of the Residence.
